Fill out the appropriate DPE Request form / select date
ASEL/AMEL Request: For all Private, Instrument, Commercial or Add-On Ratings. Including Adding-On Single Engine or Multi Engine ratings to existing certificates.
CFI Request: CFI Initial, including Sport Pilot CFI Initial exams.
Administrative Request: For items such as ground instructor ratings, SOE, SIC. Please use the Admin DPE-Request form. Date options provided on
availability.
Special Medical Request: Get the process started, involves a special process, which will be provided.
